Newsome Band 2022/2023 Forms

Please take some time to adequately review all of the forms required for participation.  These forms are to help ensure a safe and memorable experience as a member of the Newsome Band program.  Start first with the School District of Hillsborough County Clearance for Participation.

SDHC Clearance for Band Students
Brass, Woodwinds and Percussion Only - NO Physical Required

1. Watch this WALKTHROUGH VIDEO for instructions

2. Visit www.nfhslearn.com and search courses for the following training videos:
  • Concussion in Sports - What You Need To Know
  • Heat Illness Prevention
  • Sudden Cardiac Arrest
​
3. Combine video certificates into one document.
​
4. Create an account at https://athleticclearance.fhsaahome.org/ 
​
5. Upload Parent ID, Student Video Certificates, and complete medical information.
SDHC Clearance for COLOR GUARD  Students
COLOR GUARD STUDENTS ONLY - PHYSICAL REQUIRED​
 
MAKE SURE TO SELECT
"Band Auxiliary" when creating your Home Campus Account.

2. Take this form to the doctor for a physical (SDHC Physical Form)
​
3. Visit www.nfhslearn.com and search courses for the following training videos:

Instrument and Uniform Rental Agreements will be available at Band Camp.
